This webinar will discuss the key components of ensuring your health center’s intake forms are LGBTQ+ inclusive. The webinar will offer concrete recommendations to include on patient registration forms, as well as provide thoughtful questions to consider as a health center. Participants will also be able to process alternate solutions when changing forms and record fields are presented with systemic barriers. Creating LGBTQ+ Inclusive Intake Forms will increase participant knowledge of LGBTQ+ affirming best practices and participants will receive a list of resources to utilize for continuous learning.
About the Trainer:
Elise Berrier (she/her/hers) is a Clinic Training Specialist for SHIFT NC. Elise provides trainings for health centers all over North Carolina on a range of sexual and reproductive health topics, ultimately helping them to adopt national best practices for providing teen-friendly sexual and reproductive health care. Prior to joining SHIFT NC, Elise worked as a community health educator for Planned Parenthood Hudson Peconic and wrote the health system’s first interdepartmental LGBTQ+ Inclusivity Work Plan. Elise identifies as queer and specializes in helping health clinics, schools and systems become more queer- and trans-affirming.
