Clinic Improvement and Linking Specialist

Reports to: Director of Public Health Strategy

Classification: Full-time, Exempt

Funding Period: Budgeted through September 2019, with potential to continue based on funding

Position Summary:

This position will be based in Durham and collaborate with other members of SHIFT NC’s quality improvement team to provide training and technical assistance to partner organizations. Specifically, this position will assist on the All Together Now initiative in Durham and on other health care quality improvement projects, as assigned. The Specialist will use data to inform strategy and drive quality improvement approaches with health center partners. The Specialist will plan, organize, and facilitate training and technical assistance support to specific health center and youth-serving partners in order to increase teen access to high-quality sexual and reproductive health services and integrate best practices for adolescent health care.

Essential Responsibilities:

  • Develop and maintain relationships with senior health center leaders and key staff.
  • Assist/support health center leaders in prioritizing focus of improvement efforts and effectively and efficiently implementing improvement efforts.
  • Plan, coordinate, deliver, and monitor training and technical assistance to health centers.
  • Collaborate with Evaluation Specialist and health center staff to identify, develop and maintain systems for collecting and reporting key data and information.
  • Collaborate with Evaluation Specialist to ensure that project partners fully complete evaluation related activities (needs assessment, staff/provider survey, youth survey, quarterly dashboard, etc.) and that they are submitted in a timely manner.
  • Organize regular internal team meetings with health centers to review data and progress towards improvement goals and to address any current or outstanding evaluation tasks.
  • Create and maintain linkages between youth serving systems and health centers.
  • Plan and deliver training to youth serving systems to build referral capacity.
  • Coordinate with project evaluator to develop and maintain systems to collect, report, and use data related to youth-serving system’s referral capacity.
  • Assist with monitoring the progress of health centers and youth-serving partners, including implementation of qualitative and quantitative assessment processes.
  • Work with other staff and partners to develop training materials and resources.

Education and Background:

  • Master’s-level degree in related field preferred
  • 5-10 years in teen pregnancy prevention, sexual health, or related field
  • 3-5 years working with or in health care settings, experience with Title X settings preferred
  • 3-5 years using quality improvement strategies
  • 3-5 years providing guidance on data collection, interpretation, and evaluation
  • Prior experience providing training to adult learners (e.g. health care staff, health educators, social workers, and other youth serving professionals)
  • Prior experience working with diverse community members and groups
